Is My Penis Getting Smaller With Age? What You Can Do

· 5 min read

A patient once sent us an email marked urgent: "My penis is getting shorter with age. How can I slow this down?" It is a more common worry than most men admit, and it has a real physical basis.

The good news is that there is no need to accept it as natural and do nothing. Age-related changes aren't inevitable. They can be managed.

Why it happens

  • Reduced blood flow: with age, arteries stiffen and narrow. Less blood reaches the penis, and erections become less full. Over time this can lead to a functional shortening.
  • Falling testosterone: lower levels affect erection quality and the health of penile tissue.
  • Weight gain: fat above the pubic bone can hide part of the shaft, so the penis looks shorter than it is.
  • Scarring: conditions such as Peyronie's disease can cause shortening and curvature.

How to slow it down

If your vascular health is protected, this process can be slowed down considerably.

  • Exercise regularly to keep your blood vessels healthy
  • Stop smoking, which damages small blood vessels directly
  • Have your testosterone checked, and treated if it is low and causing symptoms
  • Keep your weight, blood pressure, cholesterol and blood sugar under control
  • Stay sexually active: regular erections help keep penile tissue healthy

When treatment can help

If erections are weaker, shockwave therapy (ESWT) and regenerative treatments target blood flow. If testosterone is low, testosterone replacement therapy can help in the right patient.

If a fat pad above the penis is hiding length, pubic fat reduction can reveal it, and our penile enlargement page explains the other options.

Frequently asked questions

Does the penis really shrink with age?

It can look and feel shorter, mainly because of reduced blood flow, lower testosterone and weight gain around the pubic area. Much of this can be slowed down.

Can lost length be regained?

Sometimes. Improving blood flow, treating low testosterone and reducing pubic fat can all restore visible length. An assessment shows which applies to you.
