Penile Lengthening Surgery (Suspensory Ligament Release)
Part of the penis sits inside the body, anchored to the pubic bone by the suspensory ligament. Penile lengthening surgery releases this ligament so that more of the shaft moves outside the body, increasing its visible length.
It is a well-established procedure, but expectations matter: the gain is mainly in flaccid length, and the result depends on careful aftercare. We explain what is realistic for you before any decision is made.

Who Is It Suitable For?
If your erections are not as strong as they used to be, we assess and treat that first. A longer penis with weak erections does not solve the real problem.
- Men who want more visible length and have healthy erections
- Men whose penis appears short because it sits close to the pubic bone
- Men willing to follow a traction programme after surgery
- Men who also want girth, as it can be planned together with filler
How Is the Surgery Performed?
- Performed under general or spinal anaesthesia
- A small incision is made at the base of the penis, in the pubic area
- The suspensory ligament is carefully released from the pubic bone
- The procedure takes around one hour
- Most patients go home the same day or the next morning
What Results Can You Expect?
The gain is mostly in flaccid length and typically measures in the range of one to a few centimetres. Erect length changes less. Results vary from person to person, and we will give you a personal estimate after examining you.
Recovery and Aftercare
- Return to desk work within a few days
- A penile traction device is used for several months to stop the ligament from reattaching and to protect the gain
- Sexual activity can usually resume after around 4–6 weeks, once your surgeon confirms
- Avoid heavy lifting and intense exercise for a few weeks
Risks and Side Effects
- Swelling and bruising in the first weeks
- The erect penis may point at a lower angle than before
- Infection or slow wound healing, which are uncommon
- Some or all of the length gain can be lost if traction is not followed
Frequently Asked Questions
Does lengthening surgery make the erect penis longer?
The main gain is in flaccid length. Erect length changes less. We will tell you what to expect in your case after an examination.
Can lengthening and girth enhancement be done together?
Yes. Many men combine ligament release with hyaluronic acid filler, or with pubic fat reduction, so that length and girth are addressed in one plan.
Will it affect sensation or erections?
The ligament release does not involve the nerves or erectile tissue, so sensation and erections are not expected to change. The angle of the erection may become lower.
How long do I need to stay in Istanbul?
We usually plan a stay of about a week for the operation and the first check-up. Follow-up continues remotely after you return home.
