Nocturnal Erection Test: Is Your ED Vascular or Stress-Related?

During sleep, healthy men have several erections a night, mostly during REM sleep and in the early morning. Because the conscious brain is switched off, stress and performance anxiety cannot interfere with them.

Measuring these night-time erections tells us how healthy the blood supply to the penis really is. It helps answer a key question: is an erection problem caused by the blood vessels, or by stress and anxiety?

Nocturnal Erection Test: Is Your ED Vascular or Stress-Related?
“Once a man has had an erection problem, the worry of "will it happen again?" can keep it going, even when his blood vessels are healthy. So we measure the quality of the erections the body produces on its own while he sleeps.”
Assoc. Prof. Dr. Alper Kafkaslı

Who Is It For?

  • Men who are unsure whether their erection problem is physical or psychological
  • Men who have had treatment such as shockwave therapy (ESWT) and feel they haven't benefited enough
  • Men whose erections are fine on some days and not on others
  • Planning before surgery, such as a penile prosthesis

How Is the Test Done?

  • You fit a small sensor on the penis before going to bed
  • It records the number, duration and firmness of erections during sleep
  • The data is compared with what is expected for your age
  • The test is painless and is done at night while you sleep normally

What the Results Mean

Strong night-time erections suggest the vascular system is healthy and that daytime problems are likely caused by stress or performance anxiety. Weak or absent night-time erections point to a vascular cause that needs treatment. The result guides what we do next: regenerative treatment, sex therapy, or both.

A Real Example

A 52-year-old patient told us he had benefited only partly from his treatment. His night-time erection quality, measured at 6 and 6:30 in the morning, was around 97% and 99% of what is expected for his age. His vascular health had truly improved. His remaining problem was stress during sex in the daytime, so we shifted the focus of his plan to managing that.

Frequently Asked Questions

Do I need to stay in hospital for the test?

No. The sensor is worn at night while you sleep, so you sleep in your own bed or hotel room.

Why test after treatment?

If you feel a treatment has not worked well enough, the test shows whether the vascular problem is still there or whether stress is now the main cause. The two need different solutions.
